Rochester, NY -- The Monroe County Sheriff's Office has released the name of a Rochester woman killed in a fatal crash Thursday night.
It happened on Jefferson Road just after 6:30 p.m. Deputies say a van, driven by Patrick Mulvehill, 46, of Pittsford was headed westbound without his headlights on, when he rear-ended a car. That car was pushed into the eastbound lane where it was hit by two other cars.
The second driver, Barbara Uthman, 63, was thrown from her vehicle and killed. Deputies say alcohol was a factor in the crash. Mulvehill was charged with second degree manslaughter, driving while intoxicated and reckless driving. Mulvehill was also ticketed for following too closely, passing a red light, no headlights and no tail lights.
He was arraigned in Henrietta Town Court and remanded to the Monroe County Jail in lieu of $50,000 cash bail or $100,000 bond.
